Dr Eve identified a market gap for something far more authentic than typical BDSM role play while working as a nurse, leading her to open her first fetish clinic in Dresden before expanding to Berlin last year.
Realistic Sessions and International Clientele
Sessions at the clinic are meticulously designed to mirror real medical appointments, involving examinations using devices such as ECG monitors and ultrasound machines. What follows, however, is determined by the patients, who can request anything from dental procedures to more invasive scenarios. The clinic attracts international clients willing to travel from places like Britain and France for these 'treatments', with sessions starting at around 250 Euros per hour and often lasting all night.
Meet Dr Lana van Orten: A Nurse with a Passion
One of the clinic's practitioners, Dr Lana van Orten, 46, works part-time as a nurse at a hospital and spends the rest of her time as a fetish doctor. She offers a range of treatments, including catheterisation, prostate exams, and saline infusions, promising to cater each session to her clients' needs. Dr Lana emphasises that she does not offer sex, only treatments and examinations, and remains fully clothed during sessions without administering medication beyond vitamins and saline solutions.
Dr Lana explains: 'Most of my clients want normal clinical procedures like ECGs, ultrasounds, blood samples...but also a lot of anal examinations, enemas, coloscopy, urethral catheter or dilation. And it comes together most of the time with a role play, where I have to be the strict doctor.'
Patient Demographics and Psychological Insights
Dr Lana's patients come from all over the world, aged between 20 and 85, with about 98% being men and the rest women or couples. She believes many have medical fetishes stemming from traumatic medical experiences in childhood, allowing them to re-enact those events in a safe, controlled environment. Dr Lana herself traces her interest to a hospital admission at age seven, describing it as pleasant and safe, which planted the seeds for her later clinic fetish.
A Growing Trend Among Healthcare Professionals
Dr Lana estimates that around half of her colleagues at the clinic are doctors or nurses, drawn to blending their medical expertise with BDSM role play. She notes that financial incentives may play a role, given the high session fees, but many are passionate about creating authentic experiences. Dr Jonathan Noir, another staff member, has also left his healthcare career to join the clinic, treating patients from Slovakia, the US, and Russia.
Dr Lana reflects: 'I think they like these two worlds of treating people in hospitals and in the world of BDSM. They like that they have the techniques, and I think they like putting those worlds together to create a good session for their patients.'

Despite the unconventional nature of her work, Dr Lana stresses the importance of trust and safety, allowing clients to share their deepest fantasies. She discovered her passion for role play in 2014 after ending a long-term relationship and joined the Berlin clinic in 2024 after attending an open evening. For her, the authentic clinical environment is key, and she takes pride in helping patients feel safe and fulfilled.
