Chatbots, especially OpenAI's now-retired GPT-4 model, often respond with mystical, sycophantic language, implying users have heightened spiritual importance or are communicating with cosmic beings.

Morrin and a colleague initially observed patients using large language model AI chatbots to validate their delusional beliefs. Media reports from April last year further highlighted cases where individuals had delusions affirmed or amplified through AI interactions, prompting this research. While some scientists caution that media may overstate AI's role in causing psychosis, Morrin appreciates the rapid attention these reports bring, as academic research struggles to keep pace with AI's swift development.

Terminology and Risk Factors

Morrin advocates for more cautious phrasing than "AI psychosis" or "AI-induced psychosis," suggesting "AI-associated delusions" as a more agnostic term. This is because current evidence links chatbots to delusional thinking but not to other psychotic symptoms like hallucinations or thought disorder. Researchers, including Dr. Kwame McKenzie of the Center for Addiction and Mental Health, emphasize that those in early stages of psychosis development are at higher risk, as psychotic thinking evolves non-linearly and not all pre-psychotic individuals progress to full delusions.

Dr. Ragy Girgis, a professor of clinical psychiatry at Columbia University, warns that chatbots could worsen "attenuated delusional beliefs," where individuals are not fully convinced of their delusions. The worst-case scenario is when these become full convictions, leading to irreversible psychotic disorders. Historically, vulnerable people have used media to reinforce delusions long before AI, but chatbots offer faster, more concentrated reinforcement due to their interactive nature, potentially speeding up symptom exacerbation, as noted by Dr. Dominic Oliver of the University of Oxford.

Chatbot Performance and Safety Measures

Girgis's research found that paid and newer chatbot versions respond better to delusional prompts than older ones, though all perform poorly. This variability suggests AI companies could program safer chatbots to identify delusional content. In a statement, OpenAI acknowledged that ChatGPT should not replace professional mental healthcare and collaborated with 170 mental health experts to enhance GPT-5's safety, though it still gives problematic responses to mental health crisis prompts. Anthropic did not comment on the findings.

Creating effective safeguards is challenging, Morrin explains, as directly challenging delusional beliefs can lead to social withdrawal. Instead, a balanced approach that understands the source without encouragement is needed, which may be beyond current chatbot capabilities. The study authors strongly advocate for clinical testing of AI chatbots in conjunction with trained mental health professionals to mitigate risks and ensure safer interactions for vulnerable users.
